WHAT IS A "GUARANTOR"
The guarantor is the person in the household that is financially responsible for payment of the bill. An SHMG guarantor bill may include services provided by one or more SHMG providers to one or more family members.
MAILING SHMG PAYMENTS
Payments can be sent to the Baltimore, Maryland, address on your statement. This is the central processing location for our local bank. Sending your payment to this address ensures prompt posting to your SHMG account. A return envelope should be provided with your statement for your convenience.
You should always bring your up-to-date insurance information, and proof of identification with you every time you come to an SHMG provider for services. If you do not have all of the information with you, please call our SHMG Customer Service Department at 570-326-8723 or toll free at 1-800-223-3479 as soon as possible after your visit.
After receiving SHMG provider care, bills are sent to your primary insurance carrier. After your insurance company processes your claim, they send you an "Explanation of Benefits" telling you the action taken on your bill. If there is a balance, we will bill any secondary or additional insurance carrier you have previously supplied. Once all insurances have been billed, any balance is dropped to the guarantor's responsibility for payment. If you have questions about services not covered by insurance, it is your responsibility to contact your insurance carrier. The insurance contact numbers should be printed on the back of your insurance card(s).
You are responsible for non-covered services, deductibles, co-payments, and co-insurance balances not covered by your insurance(s). SHMG will mail you a bill listing your responsible balances. The guarantor bill will list the date(s) of service, the provider, type of services, and insurance payments or adjustments.

We accept Visa, MasterCard, Discover, and American Express payments by phone, mail and online. You may also mail us a check to the address printed on your statement.
